After reading these directions, click the 'Rate Calculator' button below and you will be taken to the Rate Calculator page to determine your benefits and premium rates.
After you have completed the rate calculator, you will continue to the application form.
Once the application has been completed, you will be asked to choose a payment mode. These include automatic bank draft or direct bill quarterly, semi-annual or annual. Once you have chosen the payment mode, please follow the payment mode directions.
